1. What Is LUF?

Luteinized Unruptured Follicle, or LUF syndrome, is a condition where a mature follicle in the ovary undergoes hormonal changes similar to normal ovulation, but it does not rupture to release the egg.

In a normal menstrual cycle, the follicle grows, an LH surge occurs, and the follicle ruptures to release an egg.

In LUF, the follicle grows and starts producing progesterone, but the follicle wall remains intact. So, ovulation does not actually occur. This is why it is also sometimes referred to as “silent ovulation.”

2. Causes of LUF

The exact cause is not fully known, but some factors linked to it are:

Hormonal imbalance: Weak or inadequate LH surge

Pelvic conditions: Endometriosis and Pelvic Inflammatory Disease (PID)

PCOS: Women with PCOS may have a higher chance of LUF

Medications: Long-term use of NSAIDs such as ibuprofen and naproxen

Stress and lifestyle factors

3. Symptoms

LUF usually has no obvious symptoms. Most Women only find out when they face difficulty conceiving.

Regular periods may still occur

No mid-cycle ovulation pain or cervical mucus changes

Infertility despite regular cycles

4. Diagnosis

Doctors diagnose LUF using:

Transvaginal Ultrasound: Shows a large follicle that does not collapse after the expected ovulation day. It may appear similar to a corpus luteum but without follicle rupture.

Serum Progesterone Test: Progesterone levels rise, which suggests luteinization, but ultrasound shows no follicle rupture.

Ovulation Predictor Kits: May show a positive result, but actual egg release does not occur.

5. Treatment Options

LUF itself is not usually dangerous. Treatment focuses on restoring normal ovulation, especially for women trying to conceive.
Ovulation Induction: Medications such as Clomiphene Citrate or Letrozole

hCG Trigger Injection: Helps the follicle rupture at the appropriate time

IUI / IVF: May be considered if other treatments do not work

Lifestyle Changes: Managing stress, maintaining a healthy weight, and reducing inflammation

6. Can You Get Pregnant With LUF?

Yes, pregnancy is possible, but the chances may be lower with untreated LUF. With appropriate monitoring and treatment, many women with LUF may be able to conceive successfully.
